Terms of Service

Effective September 12, 2026

These Terms of Service ("Terms") govern your purchase and use of reports, data, templates, and related services (the "Products") provided by StarTorch LLC, a Wyoming limited liability company ("StarTorch," "we," "us"). By placing an order or using our website, you agree to these Terms.

1. The Products

StarTorch provides business information reports containing commercial insurance reimbursement benchmarks derived from publicly available price-transparency disclosures, including machine-readable files published under the federal Transparency in Coverage rule. Products are delivered digitally by email, generally within 24–48 hours (business days) after your order and payment are confirmed.

2. Eligibility

The Products are intended for use by healthcare professionals, practices, and businesses located in the United States. You must be at least 18 years old and able to form a binding contract.

3. No Advice; No Guarantee of Outcomes

The Products are informational tools only and do not constitute legal, tax, financial, billing, or medical advice. Negotiation templates are provided for your convenience; you are responsible for reviewing them and for any communication you send. StarTorch does not guarantee any reimbursement rate, contract change, revenue increase, or other outcome.

4. Data Accuracy

Benchmarks are computed from data published by third parties, which may contain errors, omissions, or outdated information. We apply reasonable processing and quality controls, but the Products are provided "as is" and "as available," without warranties of any kind, express or implied, including merchantability, fitness for a particular purpose, and accuracy.

5. License and Acceptable Use

Upon payment, we grant you a non-exclusive, non-transferable license to use the Products for the internal business purposes of your own practice. You may share the Products with your professional advisors and with payers in the course of your negotiations. You may not resell, sublicense, publish, or redistribute the Products or build a competing dataset from them.

6. No Protected Health Information

You agree not to submit protected health information (PHI) or any patient or client information to StarTorch. Our Products do not require it. Any such information received in error will be deleted.

7. Orders, Payment, and Renewals

All prices are listed in U.S. dollars. Orders are placed by email to contact@startorch.net. Before you pay, we confirm that sufficient public rate data exists for your requested county; payment is then collected through a third-party payment processor. Single County Snapshot and Negotiation Dossier purchases are one-time payments. The Annual Benchmark Renewal is billed once per year until cancelled; you may cancel at any time before your next renewal date by email. Refunds are governed by our Refund Policy.

8. Intellectual Property

StarTorch retains all rights in its software, methodology, compilations, report designs, and trademarks. CPT® is a registered trademark of the American Medical Association. Payer names are trademarks of their respective owners and are used for identification only; no affiliation or endorsement is implied.

9. Limitation of Liability

To the maximum extent permitted by law, StarTorch will not be liable for any indirect, incidental, special, consequential, or punitive damages, or for lost profits or revenue. StarTorch's total liability for any claim relating to the Products will not exceed the amount you paid for the Product giving rise to the claim.

10. Governing Law

These Terms are governed by the laws of the State of Wyoming, without regard to its conflict-of-law rules. Any dispute will be resolved in the state or federal courts located in Wyoming, unless otherwise required by applicable law.

11. Changes and Contact

This Privacy Policy explains how StarTorch LLC ("StarTorch") collects, uses, and protects information about customers and visitors to our website.

We do not collect protected health information (PHI) or any information about your patients or clients, including names, dates of birth, diagnoses, treatment details, claims, or insurance member IDs. Please do not send it to us.

How We Use Information

Sharing

We do not sell or rent personal information. We share information only with service providers who help us operate our business (such as payment processing and email delivery) under confidentiality obligations, or when required by law.

Cookies and Tracking

This website does not use advertising cookies or third-party tracking pixels.

Retention and Security

We keep order records for as long as needed to provide the Products and meet legal, tax, and accounting requirements. We use reasonable administrative and technical safeguards to protect information.

Your Choices and Rights

You may request access to, correction of, or deletion of your personal information by emailing contact@startorch.net. Residents of certain states may have additional rights under applicable state privacy laws, and we will honor verified requests as required.

Our Products are intended for businesses and professionals and are not directed to anyone under 18.

Refund Policy

Effective September 12, 2026

Our reports are digital products prepared for your specific county. We want every customer to receive exactly what they paid for. This policy applies to all purchases from StarTorch LLC.

Full Refund — Coverage Not Available

If we determine that public rate data for your requested county and CPT codes is insufficient to produce a reliable benchmark, we will notify you before delivery and issue a full refund.

Full Refund — Late Delivery

Reports are delivered to your inbox within 24–48 hours (business days) after your order and payment are confirmed. If your report has not been delivered within 3 business days of confirmation, you may request a full refund.

Errors in Your Report

If, within 7 days of delivery, you notify us that your report contains a material error (for example, the wrong county or CPT code), we will correct and reissue the report at no charge or, if we cannot, refund your payment in full.

After Delivery

Because reports are delivered digitally and in full, purchases are otherwise non-refundable once delivered, except where required by law.

Annual Benchmark Renewal

You may cancel your renewal at any time; cancellation prevents future charges. If you are charged for a renewal and request a refund within 14 days of the renewal date, before any refreshed report for the new term has been delivered, we will refund that renewal charge in full.

How to Request a Refund

Email contact@startorch.net from the address used for your order, with your order date and the product purchased. Approved refunds are returned to the original payment method, generally within 5–10 business days depending on your bank.

Zero PHI & Data Sources Statement

100% Zero PHI

StarTorch LLC does not create, receive, maintain, or transmit protected health information (PHI) as defined under the Health Insurance Portability and Accountability Act of 1996 (HIPAA). Our products are compiled solely from publicly available pricing disclosures and public provider directory information, together with non-clinical business information supplied by our customers (such as county, CPT codes, and their own contracted rates).

StarTorch is not a covered entity and does not perform functions on behalf of covered entities that would make it a Business Associate. No Business Associate Agreement is required to purchase or use our products.

Data Sources

Regulatory Context

Section 203 of the Consolidated Appropriations Act, 2021 (CAA 2021 § 203) requires group health plans and health insurance issuers to perform and document comparative analyses of nonquantitative treatment limitations (NQTLs) applied to mental health and substance use disorder benefits. StarTorch references this requirement for context only; our reports do not constitute a parity compliance analysis or legal advice.

Methodology Summary

Rates are filtered to relevant psychotherapy CPT codes and behavioral health provider types, associated with counties, and summarized into percentiles (including the 10th, 50th, and 75th) and payer-level medians. Each benchmark retains a reference to its payer, source file, and reporting period.
